You have the right to receive a Good Faith Estimate of the expected charges for your scheduled services. You may also request a Good Faith Estimate before scheduling services.
Your Good Faith Estimate will reflect the services that are reasonably expected based on the information available at the time the estimate is prepared. Because the frequency and duration of counseling or mental health services may vary according to your individual needs, your actual number of sessions may differ from the initial estimate.
A Good Faith Estimate is not a contract and does not require you to receive services from Grafted Counseling and Wellness LLC.
If you receive a bill from a healthcare provider that is at least $400 more than the amount listed on that provider's Good Faith Estimate, you may be eligible to use the federal patient-provider dispute resolution process.
You may also contact the healthcare provider to discuss the bill and determine whether the charges can be resolved.
